HSE Diving Medicals Health and Safety Executive (HSE) INITIAL DIVING MEDICALS
Please note that it is a requirement of the HSE medical examination that a declaration from the diver's General Practitioner (GP) is obtained to confirm the presence or absence of any significant past or present medical condition, together with a list of current medication (if any). The diver is required to bring this to the first (initial) medical examination. The GP may email this brief report to: email@example.com
Even if there are no past or present medical conditions this must be available at the time of the initial examination
